There are many selling points that arrange the necessary information to be printed on a metal tag. However, if you cannot find it in your pet store, try hardware stores. They are a good place where you can have things engraved. Ask if the staff there can imprint your pet's info onto a small tag that you can attach to the collar, etc.For more luxury pet collars (those involving precious metals) you might need a store specializing in engraving, a jeweller or other similar establishment. In this case, you might have to pick out materials, style, and go over these details with the staff to make sure you get exactly what you want. 

    Another option is to search online stores for personalized collars. There are many websites that are dedicated only to the personalized cat collars, designer cat collars, reflective ones etc. You can also try to design and personalize a cat collar on your own with a pen and a good laminate. Make sure the edges of the laminate are sealed so that your pet's information doesn't get worn away by the elements.
